What could cause these defects?

Have you checked the drawing? It appears you have connected two lines. Send us the g code to see if it is a glitch. Other than that, it looks like you have a lousy work clamp connection—or way too fast ipm. The consumables condition is also a factor.
OR Torch cable too short and shifting. Burr on the X axis (I am presuming that is left to right)
Quite a few possibilities.
Also check the torch mount and the bearings of the z-axis.
